Request for Proof of Amateur Activity

This form is for those CHARL Members who would like to renew their RSL/ROC with the NTC and one of the requirements is a Proof of Amateur Activity and Certification of bonafide membership with CHARL for renewal purposes.

Requirements:

For Current Members:

1. Fill up the Member’s Information Form here: https://forms.gle/JSRkcVnnvZ9jVjWi9
2. Scan or clear photo of Radio Station License (RSL) or Radio Operator’s Certificate (ROC).
3. Proof of payment of current and previous year’s Annual Dues. Please contact the Club’s Treasurer, DV7BMY-Kimoy, for the amount and payment of dues. You may also pay online: https://forms.gle/DC9KPdnHAZQGfjAfA
4. Proof of Payment of ₱50.00 fee for the certification.

For New Applicants:

1. Fill up the Application for Membership Form here: https://forms.gle/JSRkcVnnvZ9jVjWi9
2. Pay the Membership Fee of P300.00 here: https://forms.gle/DC9KPdnHAZQGfjAfA
3. Or you may pay directly to our Club Treasurer, DV7BMY-Kimoy.
4. Scan or clear photo of Radio Station License (RSL) or Radio Operator’s Certificate (ROC).
5. Proof of Payment of ₱50.00 fee for the certification.
